Whether you are coming in for your first eye exam or updating your glasses prescription, our Patient Center is here to help you feel prepared and informed. Below, you will find important information about accepted insurance, what to bring to your appointment, and how we support your vision care before and after your visit.

What to Bring to Your Appointment 

To help your visit go smoothly, please bring the following items: 

  • A valid photo ID 
  • Your current vision insurance or medical insurance card 
  • A list of any medications you are taking 
  • Your most recent glasses or contact lenses (if applicable) 
  • A referral, if required by your insurance provider 

If you are a new patient, we recommend arriving a few minutes early to complete your intake paperwork. 

Insurance Plans We Accept

We accept a variety of medical and vision insurance plans, including Medicaid, Fidelis, HealthFirst, UnitedHealthcare, Blue Cross Blue Shield/Anthem, and Medicare.

CareCredit is available for patients without active coverage. 

If you are unsure whether your plan is accepted, please contact us, and we will be happy to verify your benefits and explain your coverage.

Forms & New Patient Information

Our front desk team will provide any required forms when you arrive. If you would like to complete your paperwork ahead of time, please call our office to inquire about pre-registration options.
